I agree with you except for the competition across state lines.  That, in my opinion, is playing in to the Democrats' plans to nationalize health care.  It would require federal regulation of insurance and strip the states of much of their powers to regulate the industry.  I think we need more state involvement - when you start opening up competition across state lines, it just begs for a Federal Department of Insurance that would promulgate the regulation that would preempt state regulation.
