I've gotten pretty good results with the Xerox Travel Scanner 100.  It's compact & lightweight and runs directly off the USB port, so you don't need a separate power supply.  It comes with the latest version of PaperPort, and scans directly to .pdf.  My only complaint is that it's sometimes difficult to get documents to feed properly.  Other than that, it does produce a very nice hi-res scan and is an all-around decent piece of field equipment.  It runs about $200.00 and you can find it at Office Max or Staples.
