I may not have worded the subject line correctly but others knew what I meant by adding them as a Certificate Holder once it was explained in the body of the post.  Thanks.  Back to my question:

For those who have done this and it's been a courtesy from your insurer, please let me know who your provider is either publicly or via email.  It took me a week to get this quote which makes us look like we're not on the ball.  AND it's a pretty high fee considering others are getting it free. I've asked our broker if this is the insurer's fee or their fee.

I'm glad this site is here for these kinds of topics or I'd be relying on our broker's word or trying to ask around to my fellow vendor managers.
