I have worked out a system that works great for me using Microsoft Works database and Paperport.  It takes a few minutes to type my reports in the database and then I can "print" it to Paperport.  I use a scanner at the record valt and load my scans into Paperport.  I can then combine the report & scans into one document.  you can print, email or fax from Paperport and it has an indexing feature that will allow you to search your documents.  It sounds a bit complicated, but it really only takes a few minutes.  You're welcome to email me if you want any more info.
