That is incredibly ludicrous. I don't even know how you can issue a policy without the recording information. The volume and page (or instrument number), and the filed date and time need to be on the policy.
This is clearly their fault for issuing a policy without the proper information. They obviously didn't wait for the title update, so what if you had found intervening liens? Sounds like reckless underwriting practices to me.
I would call them and tell them that you didn't do anything wrong and perhaps suggest that you get their underwriter's opinion. I don't think any underwriter would support their agents issuing policies without an update or filing information on the insured mortgage.
