We have significant problems with mortgage deeds that are sent to the wrong town clerk's office for recording.  Most often, that "wrong" town clerk sends the unrecorded mortgage back to the address on the mortgage deed with a little yellow sticky noting that the mortgage is unrecorded because it was sent to the wrong town.  Those mortgages rarely get redirected to the correct land records, thus they never get recorded.
