We have a MS Word form that we designed. We use that form for the report. We use a small $40 scanner that uses the computer as its power source for deeds etc. We put everything together into a pdf file and email them to the client. All of our clients prefer email over faxing now.  We also return their order form with their order. Our report form has an area for our fee. An SOT friend gave me a copy of an Access program he designed for abstractors. I am hoping to eventually have the info on my form feed into that program.
