Thanks for your input.  The research I've been able to come up with indicates about a 5% error rate at the top end and reducing down to about 2% as time is added.  Errors can be categorized in various areas, from typos to missed conveyances...the entire gamut.  I'm trying to document a point...the more pressure to provide high volume with time constraints, the higher the error rate.  I'm sure some e&o insurance actuary has this figured out.
