Before I read the comments I immediatly thought that this was straight out of a Quality Assurance process.  I was a QA Director for years and would use many methods to qualify suppliers and solicit feedback from suppliers and customers alike including surveys.

The intent was continuous improvement of the entire process from womb to tomb.

Without seeing the document - I will assume that is their intent.
