Yes, errors are human but what matters is the manner in which the error is corrected. If it is fixed in a timely manner that is one thing but some of the post I read speak a different story.
I do agree with adding some sort of location indicator because as you say reputations of companies are at stake. I worked for a company once that was confused with another that had just gone out of business the week before. I got some really wonderfull phone calls for a while!!
