I would be tempted to fax back on their order in big black marker - pay me for that last (however many) and attach my invoice for the totally amount. 

ew, better yet....send back a fax 5 minutes later saying the exam is complete and upon payment of xyz you will send the commitment.  Dont forget to add in for the knew order too. 

Fax repeatedly until paid.
