The problem with people "not wanting" health insurance is that when they have a serious health problem, they can't pay for it. That, unfortunately, means that they get their treatment and the doctors and hospitals have to write it off - and charge the rest of us more to make up the difference. It causes all of us to pay more.
Insurance is a numbers game. If they cover more people, rates are lower - they spread their risk over more payers. Think of it in terms of auto insurance - if all of the safe drivers opted not to get insurance, the poor drivers would all have to pay so much that many couldn't afford it. While many may simply think "who cares?"... it would cost the safe drivers more because when they get hit by a poor driver, there wouldn't be any insurance to pay for it.
And, speaking of auto insurance - it is required in most states, I believe. So, obviously the government can mandate insurance coverage. I don't think there is any Constitutional issue there.
