If the mortgage has been satisfied there are a number of ways to deal with it. Get a satisfaction/release from the lender if possible. If that is not possible some states permit the borrower to record an affidavit with written evidence of the final pay off. However, if you can not get a satisfaction/release states such as Connecticut have an expedited civil procedure whereby the court will discharge an invalid lien/mortgage. If all else fails there is an Action to Quiet Title.
