  Since I spend all 9 hours of the work days at the San Mateo County Recorder, I see tech failures in their systems almost every day. 

  Often, something that starts out small, becomes big hours or days later as it affected more systems that they were aware.  On some occassions, new software affects other systems.  Recently, a new program was delaying the ability of another to put the scanned documents onto the public record.  I discovered it when a client wanted a copy of a recent record and it was unavailable.  It's a problem that could have continued for weeks or months longer had I not spotted it and foced them to look at it (they are very reluctant to believe that I k now better than their unionized tech staff).

  Indeed, in every case, implementation and understanding by humans is at the root of the errors.
