Not sure that removing "systemic" from regular, ongoing and frequent blunts any of my points at all. 

Well, think of it this way-- title searchers miss items during searches on a regular, ongoing and frequent basis (arguably at least, depending on how you define frequent I guess.).  Is it time to put 60 Minutes on the case to see why that is occurring?  Or can we just assume that title searches didn't want to make those mistakes, and they were caused by human error?

why would an investigative journalist be the biggest victim?

Yes, but if there's no systemic wrongdoing and no victim, there's isn't much meat on the bone.
