Not sure that removing "systemic" from regular, ongoing and frequent blunts any of my points at all.  I'll stipulate to adding the modifier of "potential systemic" errors and leave the rest and my postings still read just fine.  

Motive for digging deeper:   why would an investigative journalist be the biggest victim?
