Minnesota has a Title Standard on this subject.  If the lender has received a Deed in Lieu and then gives out a Warranty Deed, the mortgage is extinguished.  If, however, the lender gives a Quit Claim Deed, the lien must have a release filed, or it remains intact and is presumed to be assumed by the new owner.
