Maybe it's not part of the document, but if you're trying to release a document recorded in that office, I feel like you need to put the exact book and page number that the office has it recorded under.
It would make things a little simpler for me : ) today I had to write up 4 additional mortgages with notes about the releases because the releases gave the wrong page number, ugh.
