I'm attempting to fill out a checklist, under the order information section it requests "Date completed" and "Effective date" as if it wants two separate dates.   The only two dates that I see are "this search is current through 5-1-15" and "Due date 5-8-15 @ 3".

So I should put in the same date of 5-1-15 for the Date completed and the Effective date?
