I read your paragraph but couldn't figure out why you had an http address under the message....it didn't seem to match your message.  I have since figured it out.  Your long signature was obviously put there for a reason.

 "Pricey" to me is when something is charged over its worth.  That is just my opinion.  To me it is just one form.  If you have a website you get access to a program that allows you to make forms relatively easy.  There are also many software programs on the market that allow you to make your own custom forms...many have free forms that are really low in cost.  Perhaps you should search the internet and look at some other products and then price your product.
